Asthma how-to: How to use an inhaler with a spacer and mouthpiece
There are many asthma treatment options depending on your child's age and preference. Metered dose inhalers, which are commonly called puffers or inhalers, spray the medicine out so that you can breathe it deep into the lungs. Inhalers deliver medicine very quickly. Inhalers are compact and light so they can be easily taken with you wherever you go. It's very important to use your inhaler the correct way to make sure the medicine gets deposited into the lungs, where it works the best. Incorrect technique can leave some of the particles from the medicine on your tongue or throat, where it won't help at all. A spacer, or holding chamber, is an attachment that should always be used with your inhaler. The spacer holds the medicine in place so you can breathe it in easier. First, remove the cap from the bottom of your inhaler. When you get your new inhaler from the pharmacy, you need to "prime" or "waste" it by spraying the medicine into the air four times. Now, fit your inhaler into the opening at the end of the spacer. Shake the inhaler well for ten seconds. Turn your head to the side and breathe out. Close your mouth around the mouthpiece of the spacer. Push down on the inhaler once. Now the medicine is inside the spacer. Right away, take a slow, deep breath. The goal is to not hear a whistling sound. If you do, this means that you're breathing in too fast. Hold your breath for a count of 10. Now, slowly breathe out. Rinse your mouth out with water or brush teeth after using controller medicines such as inhaled steroids. Many times, the correct dose of your medicine will be to inhale two puffs. If this is what is prescribed for you, then wait one minute after the first puff, and then follow all of the steps again. Remember if you have questions, call your doctor's office, asthma care team or pharmacy. b lynch compression suture for Atonic PPH
B Lynch suture3: With the surgeon standing on the right and the abdomen opened for caesarian section (If applying after vaginal delivery, perform a laparotomy and give a transverse hysterotomy incision in the lower uterine segment) exteriorize the uterus and apply bimanual compression and observe for any reduction in vaginal bleeding. This will indicate whether the suture will be successful or not. You will need a no. 1 Polyglactin 910 suture preferably on a straight needle. Ensure the bladder is pushed down and then begin the stich in the lower uterine segment at a point 3 cms below the angle of the uterine incision to come out 3 cms above the uterine incision on the right side. Now carry the suture over the fundus to the posterior surface of the uterus. Here, take a horizontal bite, from right to left, through he posterior wall at the level of the anterior hysterotomy incision, just above the attachment of the uterosacral ligaments. In this way the suture passes through the posterior wall of the cavity to emerge on the posterior surface of the uterus on the left side. Now again carry the suture over the fundus. Now the needle is passed from a point 3 cms above the hysterotomy incision on the left side to emerge about 3 cms below the incision. All the while the assistant keeps the uterus compressed. And the suture is then pulled to compress the uterus and stop the bleeding and is then tied just below the hysterotomy incision.

Transport of Water and Salts in Plants - Science
Transport of Water and Salts in Plants The cells present in the soil dissolve in the water resulting in a salt solution when the soil is watered. The amount of water provided to the soil is quite large; the solution will be dilute salt solution. Roots of plants have some hair like structures called Root hairs originating from cells of single layer epidermis. Dissolved salts are present in the cytoplasm of these cells, also called as CellSap. Cell sap is much concentrated than the salt solution present in the soil. Cell sap is also a salt solution. However it is much concentrated than the salt solution present in the soil. Cell membrane of the root hair is a semi permeable membrane and it separates the cell sap which is the concentrated salt solution from the salt solution in the soil which is the dilute salt solution. Water flows along with solutes from the soil into the cell sap in the root hairs. Thus roots absorb water and salts from the process of osmosis. The absorbed water and the salts move to the next cell once again by Osmosis. The movement of the water from roots to the parts of the plant above the ground is called ASCENT OF SAP. Water is transported from the roots to the leaves through Xylem vessels. Water and salts from the root hairs pass into the epidermal cells then to the cortical cells then to Endodermis cells and finally into Xylem. The transport of water from roots to the leaves is aided by the physical and chemical properties of water and the composition of the walls of the Xylem vessels. These vessels are made of lignin and cellulose which have strong, attraction for water molecules. A thin continuous layer of water Is formed in the walls of xylem vessels. This is because of strong attraction (or) adhesive force between water molecules. Between water molecules there is a strong attraction (or) cohesive force due to formation of hydrogen bond between water molecules. Because of this thousands and thousands of water molecules attach to the water molecules present on the walls. Thus forming a continues column of water molecules. The water molecules already present in the water column either get displaced or moved by the newly entering water molecules, ultimately pushing water towards the leaves. Root Pressure is the pressure exerted by the water molecules absorbed by the root hairs.

Wound Measurement- Understand Wound Care
More information at http://www.woundphysicians.com, http://understandwoundcare.com or 1-877-866-7123. Vohra Wound Physicians: Healing Wounds, Saving Lives Understand Wound Care: Wound Measurement Demonstration Summary- This is a demonstration of a wound care physician exhibiting the correct measurement of a wound. Japa Volchok, DO explains how to accurately and consistently measure wounds. Volchok discusses the importance of proper measurement and documentation in the wound healing process. This demonstration is performed by a trained wound care physician for educational purposes only and should not be tried at home. Understand Wound Care: Wound Measurement Demonstration Commentary: Japa Volchok, DO In this demonstration we will be exhibiting the correct measurement of a wound. For wound measurement you will want to have a disposable paper or similar measuring device as well as a cotton-tipped applicator. The measuring device is either a paper or plastic disposable measuring tape. It is generally marked in centimeter markings with sub-markings in millimeters. A cotton-tipped applicator is useful for measuring depth and checking for any undermining. This model demonstrates a wound. By convention, the superior aspect of the wound would also usually coincide with the head. The inferior aspect would be the foot. We measure length from a head to foot direction. Width is measured from left to right or right to left. Depth is measured at the maximum location of depth in the wound. The left to right or right to left convention does not matter in terms of the documentation. It is important however, that length and width are correctly measured and documented. As the convention from head to feet for length and left to right for width are important in being able to reproduce measurements between measurers. In this particular instance, we would start off by measuring the wound in the length dimension. Where there is the maximum length of this wound of approximately 5.7 centimeters. The width would then be measured at its maximum point of width which is 2.5 centimeters. The depth we then would determine by probing the wound with our cotton-tipped applicator using the cotton end. We would probe all areas of the wound to determine where the most depth was located. It appears to be approximately in the middle of the wound. We would then slide our finger down the cotton-tipped applicator to where it is flush with the intact skin. Pinch the applicator at that point and withdraw from the wound. This can then be laid over your measuring device and the depth determined. In this particular instance, it appears to be 1.8 or 1.9 centimeters. Once the length, width and depth of the wound have been measured, it is important to then record any evidence of undermining and what the dimensions of that undermining are. In determining undermining, gently probe the wound with your cotton-tipped applicator circumferentially around the wound. You can see that over on this side of the wound we are starting to see some undermining. It appears at this location, the cotton-tipped applicator slips deeper under the wound then it does at any other point. This would be the maximum area of undermining. As you can see, by convention, the superior aspect of the wound or the head aspect of the wound would be 12 o'clock. If we then progress clockwise around a clock face, the area of maximum undermining is approximately 10 o'clock. Similar to how we measured the depth of the wound, you would want to slide your finger down the cotton-tipped applicator. Pinch it with your nail. Then, bring it up to your measuring device. In this particular instance, it appears that the undermining is approximately 1.7 centimeters. As you will recall from the last image, the undermining is located at 10 o'clock. This will be recorded as 1.7 centimeters of undermining at 10 o'clock. For more information visit http://www.woundphysicians.com or http://www.understandwoundcare.com

TURP Transurethral Resection Prostate via Penis Surgery
StoreMD™ for Physician Videos: http://store.preop.com Your doctor has recommended that you undergo a Trans Urethral Resection of the Prostate - or TURP. But what exactly does that mean? The prostate gland is a walnut-sized organ that is part of your reproductive system. It provides some of the fluid contained in semen. The prostate is located just under the bladder and behind the testicles. The urethra -- a hollow tube that carries both urine and semen to the penis -- passes through the prostate. In some men, the prostate gland becomes enlarged. Symptoms of an enlarged prostate include: * a full bladder feeling even when the bladder is empty * pain when urinating * weak urinary stream * infertility * and sexual dysfunction. To relieve you of your symptoms, your doctor feels that you would benefit from a surgical procedure called TURP. TURP is designed to relieve symptoms by reducing the size of the prostate. It is also a diagnostic procedure. Tissue removed during a resection of the prostate or TURP is routinely screened for the presence of cancer. So make sure that you ask your doctor to carefully explain the reasons behind this recommendation. Patient Education Company for info: 617-244-7591http://www.PreOp.com Patient Education Company for info: 617-244-7591 Your doctor will then... ...lift your penis upward. A well-lubricated instrument called a resectoscope is then gently inserted into the urethra. When the resectoscope reaches the back of the penis, your doctor will pull the penis downward in order to create a straight path into the prostate. Using this tool, your doctor will then scrape excess tissue from the prostate, restoring it to its normal size. Tissue removed from the prostate may be sent a laboratory for analysis. When the surgery is complete, your doctor will remove the resectoscope. Your doctor will probably ask you to wear a temporary Foley catheter. A Foley catheter is a narrow tube inserted through your urethra and into your bladder. The catheter is connected to a bag that is attached to your leg by a strap. While the Foley catheter is in place, urine will pass from your bladder into the bag. You will not need to urinate into a toilet. The nurse will show you how to change the bag when it is full. An appointment will be made for you to return to the doctor's office in a couple of days to have the catheter removed. As soon as the anesthesia wears off and you feel comfortable, you'll be allowed to leave.
Avascular Necrosis, Blood Supply Femoral Head- Everything You Need To Know - Dr. Nabil Ebraheim
Educational video describing the conditions affecting the blood supply of the femoral head. Where is the important blood supply of the femoral neck located? On the postero-superior aspect of the femoral neck. The main blood supply for the weight-bearing dome of the femoral head is the medial femoral circumflex artery (lateral epiphyseal artery). Using an awl at the piriformis fossa in children requiring intramedullary fixation of their femur fracture may endanger the blood supply and cause avascular necrosis of the femoral neck. Avascular necrosis is death of a segment of bone. High energy neck fracture •Vertical fracture of the femoral neck that has vertical obliquity of 70 or more degrees. Has a high rate of nonunion and avascular necrosis. •Displacement of the femoral neck fracture disrupts the blood supply (kinks the retinacualr vessels). Pressure from the intracapsular hematoma may affect the blood supply. Clinical picture •Hip is in flexion and external rotation •Surgical emergency in young adults to unkink the vessels. Treatment •Obtain anatomic reduction in young adults. •Open reduction if closed reduction is not possible, use Watson Jones approach between the gluteus medius and the tensor fascia latae. •Follow the patient with an MRI to evaluate AVN. •If the patient develops nonunion and the head is viable, treat the nonunion by valgus intertrochanteric osteotomy. Hip dislocations Posterior dislocation is common. Anterior dislocation is rare. Treatment Emergent closed reduction within 6 hours. Assess hip stability post-reduction. Other factors associated with hip osteonecrosis •Corticosteroid use •Alcohol abuse •Hemoglobinopathy •Coagulopathy •Gaucher disease •Pregnancy (rare). Idiopathic AVN Nontraumatic AVN of the femoral head is located anterolateral. Time period of a pendulum depends on its length | Oscillation| Physics
String up a pendulum, move the bob to one side and let go to set the pendulum into oscillations. Use a stopwatch to measure the time the pendulum takes to complete ten oscillations. Diving this time by 10 gives us the period of the pendulum i.e. the time taken to undergo one oscillation. Decrease the pendulum's length and repeat the above to get the new time period. We see that as the length becomes shorter, the time period decreases. This shows us that the length of a pendulum and its time period are related

How the Body Works : Repair of Bone
How the Body Works Repair of Bone The fracture of a bone is usually caused by direct violence or by a strong twisting strain. When a bone fractures the two fragments separate and in so doing tear the arteries in the Haversian systems that cross the fracture line. This damage results in the leakage of blood into the fracture, where it is trapped and soon clots. After a short time, the Haversian arteries go into spasm, causing the death of active bone cells not only at the fracture site, but also for some distance along the shaft. About two days after the break, the blood clot is invaded by capillaries and fibroblasts. The fibroblasts differentiate into bone-forming cells, or osteoblasts, and cells that form periosteal tissue on the outside of the bone. New tissue, called callus, surrounds the fracture and replaces the dead bone. The dead bone is absorbed and replaced by new bone, formed by the osteoblasts in the callus, which is remodeled by cells, called osteoclasts, to its original shape. This remodeling process is so effective that after a few months it is difficult to detect the fracture site.

What does renal pelvic diameter of 5mm in 5th month of pregnancy scan signify? - Dr. Shashi Agrawal
Renal pelvic diameter is a normal cut off at 5 months of scanning of the foetus. Usually in a low risk setting, a Renal pelvic dilatation, that is RPD is less than 2%, it is a normal variant, but it can be associated with trisomies or renal pelvic walls , which causes obstruction. It can also be associated with trisomies and antenatal reflux programs causing obstruction. In absence of additional abnormalities on scan, provided the screening tests are normal, the risk of trisomies are low and usually invasive testings are not required. The follow up scan after 2 months is recommended and renal pelvic diameter can be measured and one can look for any other anomalies in that time. The renal pelvic diameter usually resolves in the follow up scans and obstruction can be treated later after delivery post natal assessment is important after the childbirth to assess any reflux problems.
